Get Appointed with Coventry!
How to Become Appointed
In order for a producer to be appointed with the Nebraska Department of Insurance, Coventry Health Care of Nebraska requires the following to be completed and submitted to us. This paperwork will allow the agent to sell our Group Commercial Products. Be sure to:
- Thoroughly complete all paperwork. If any information is missing, the application can not be processed.
- Provide copies of the entire contract, including the Commission Attachment page.
- Submit a current copy of an NE state producer’s license and proof of current E&O insurance via secure email or other secure communication method.
